This week, a tornado has struck your county and you are tasked with doing an initial damage assessment / windshield survey.
Start a new message and select Standard Forms > General Forms > Damage Assessment.
Click Setup and enter your club or group.

• Jurisdiction - enter your county's name.
• Mission or Incident # - enter "July tornado".
• Leave Exercise selected.
• Event - select Tornado/Winds.
• Survey Area - enter "Center Township".
• Survey Team - enter "Team 2".
• Start Date of Event and Date of this Survey - click the box to enter today's date.
• HOUSES - Affected 10 % enter 1, Major 50 % enter 2, $ Estimate enter 50,000.
• PUBLIC BUILDINGS - Major 50 % enter 1, $ Estimate enter 250,000.
• ELECTRICAL DISTRIBUTION - Affected 10 % enter 15, $ Estimate enter 50,000.
• Comments -enter call sign, first name, city or town, county, state, mode (Winlink HF, VARA FM, VHF packet, etc.)

2 letter state abbreviation and no parentheses, please!
Example: WB0GAG, Bruce, Muscatine, Muscatine, IA, VARA HF

Save a copy for later if you wish, then click "Submit" and follow the prompts.
